Letter Seeking Doctors Signature for Handicapped Parking Permit

Dear Dr. Angelo

I am writing to ask if you could be of assistance in helping me obtain a handicapped parking permit.
I have enclosed the required form from the local police department, and have filled out all of the information about me and signed the form. What I am asking of you is to finish filling out the form as our family physician and sign off on the form to confirm the need for my handicapped parking permit.

This last time it was not. Fortune Insurance allows a copayment of $15 for use of generic drugs to fill prescriptions. They charge a sliding scale of up to $45 for name brand drugs. I am guessing my husband did not notice the charge when he picked up the prescription for me. I do not like paying for pretty little pills and fancy TV advertising.

I have enclosed an envelope already addressed to the proper persons, along with a stamp attached.
