In February of this year I signed up for a one-year membership to your club. Unfortunately, in June of this year I was part of a car accident and have been receiving physical therapy since. I have been instructed by my doctor not to use the equipment at the fitness club for the next year as this could cause more damage. I have enclosed a copy of this note.
