Dear Ms. Dean
Recently I was notified of a bill sent to my insurance company, Fortune Insurance, concerning treatments provided to me by your team of therapists after my accident. I am grateful for your attention and the work that your staff have done with me. Unfortunately, Fortune Insurance is stating that fees billed to them are outside of their pricing structure for physical therapy. They have also sent a bill for an outstanding balance of $400 for services rendered.
This last time it was not. Fortune Insurance allows a copayment of $15 for use of generic drugs to fill prescriptions. They charge a sliding scale of up to $45 for name brand drugs. I am guessing my husband did not notice the charge when he picked up the prescription for me. I do not like paying for pretty little pills and fancy TV advertising.
I wish to state again the wonderful work that your staff has done in helping me recover to my full potential. The circumstances outside of this assistance require me to ask if you could possibly coordinate all of the billed charges with Fortune Insurance so that the statement is settled.
