In common usage it means 'wrong interpretation of data' or 'wrong data'. It implies that the data has been wrongly constructed, or that the interpretation is based on a distortion of information. It can also relate to willful interpretation of data in the wrong way, or actual falsification of data.
A less common usage, but correct, is to refer to data where the information source has produced incorrect data as skewed.
