Permission Letter to School Nurse to Dispense Prescription Meds

Ruth Card, RN
Nurses Office
Hometown Middle School
Hometown CT

Dear Ms. Card

Our son James has allergic reactions to some forms of dust (including chalk), and as such has been prescribed Allegra (fexofenadine HCI) to limit the symptoms to manageable levels. I am enclosing one of his medicine bottles with 40 pills in it for his use. He takes one pill daily before school, and can if needed, take another after lunch.

This last time it was not. Fortune Insurance allows a copayment of $15 for use of generic drugs to fill prescriptions. They charge a sliding scale of up to $45 for name brand drugs. I am guessing my husband did not notice the charge when he picked up the prescription for me. I do not like paying for pretty little pills and fancy TV advertising.

As is required by administration policy of the Hometown School System, I am authorizing you to hold his medication in your office. He will come to you when it is needed and should not be a daily use in the afternoon.
Thank you for your assistance in this matter

Sincerely,
S. Leah Forbes
