Home Services Coordinator
Home County Office of Aging
Hometown DE

Dear Coordinator

Our mother, Dorothy Deubler is currently recovering from surgery at the Community Medical Hospital in West Hometown. She is expected to be released sometime next week.
Our father, Don, is our mothers primary caregiver, as none of the remaining family are nearby. Both of our parents are in their mid 80's.

This last time it was not. Fortune Insurance allows a copayment of $15 for use of generic drugs to fill prescriptions. They charge a sliding scale of up to $45 for name brand drugs. I am guessing my husband did not notice the charge when he picked up the prescription for me. I do not like paying for pretty little pills and fancy TV advertising.

Our father has stated that, for a brief period of time during our mothers recovery at home, he may be in need of in-home assistance at times so that he can be free to run errands and do the family shopping.
We have been told that your agency has a program specifically set up for this type of situation. We would like to arrange for someone to be with our mother approximately three times during the week to assist our dad with her care.
If you could please contact Don Deubler to make arrangements this would be greatly appreciated by the whole family. His number is (000)000-0000.

Sincerely
Jay Deubler
