As you are aware I have recently married Simon Roman after many years. I have decided to take my new husband's name. Because of this I have started to change over all documents and polices I currently hold. Please update my information in your files to reflect this name change from Ashley Barr to Ashley Roman.

Also, since Simon and I will be driving each others' cars, I will also need to add him to my auto insurance policy as a second driver. I have enclosed a copy of his driver's license. We also need to update my beneficiary information on my parent's policy as well.
