My daughter has been accepted this week into your school as a student. Her name is Amber Taylor. My husband and I use different last names for professional purposes and wanted you to be aware of this. Amber Taylor's father is Tom Taylor and her mother is Dian McAndrew.

I am also writing to inform you that either one or both of us will be bringing her to school and picking her up at the end of the day. As her legal parents, either or both of us should be notified in case there is an emergency.
