Letter to school concerning emergency contact information

Dear Dr. Murphy

I have been told by my son that I need to list an emergency contact person in the event I cannot be reached. I would like to list Pam Haus as the emergency contact point for our son, Matt. Pam can be reached during the day at 000-000-0000.

In the event that I am unable to be reached or come to the school, Pam has been given permission to pick up our son Matt and take him to her home in case of illness. If there is an emergency situation she has both my husbands, and my, cell phone information as well as authority to speak with any medical personnel until our arrival.
