Letter asking for quote on a scheduled weekly taxi service

Dear People
My daughter is a student at Community University. For the next month she is going to be involved in an allergy study at Franklin Medical Center. I do not want her taking public transportation at night.

She will need transportation to the hospital from the Student Union at the university on February 2, 9, 16 and 23.

She would need to be picked up at 7 PM at the university and taken to the hospital. Then she will need to be picked up at the hospital at 9 PM and taken back to the Student Union at the University.
Could you give me a flat price for these six roundtrips?
