Last week I purchased a set of Franklin cookware at your store. One of the reasons I made the purchase of this particular brand was because of the mail-in rebate offered by the company itself.
When I was ready to send in the necessary paperwork to Franklin Cookware to receive my rebate, I noticed that I needed a store rebate form. This form, which I was to have received at the time of purchase from the cashier, was not provided.
Please find enclosed a copy of the initial receipt. I would ask that you send the necessary rebate form so that I may complete this transaction.Sincerely
