Customer survey form

These forms are important to your business, either online or elsewhere.

The idea is to get useful information regarding customer opinions. For people providing high value services or products, this is particularly important.

There are two generic types of survey form: General, and user specific. The general form is often a simple yes/no series of questions which tries to get an ongoing supply of information from passing customers. The user specific form is usually more complex, from an existing customer, regarding a range of topics.

Feedback is extremely valuable.

It's a direct link to your clients.

Considerations for survey forms

You have to make sure you're asking what you need to know in a way it will be usefully answered. Some people aren't very forthcoming with their opinions, and you need to target your subjects.

You must provide space for comments, not just tick boxes. These comments are an opportunity for clients to say what they think, and at least some will.

The example below is for a wholesaler, asking his retailers about the service they're receiving. As you can see, this survey form is all relevant business. The wholesaler is conducting a review of his business operations, and wants specific information. This is a personalized form to existing clients.
